AGREEMENT FOR CREDIT REPAIR SERVICE
This AGREEMENT is entered into between Crystal Clear Credit Management, LLC (hereinafter "CCC"), 865 Cooperlanding Road, PMB 113, Cherry Hill, NJ 08002 and "Your Name" (hereinafter ""Client"") for the purposes of repair of"Client"s three major credit reports of inaccurate and/or negative information. The terms of the agreement between CCC and "Client" are as follows:
"Client" AGREES TO COOPERATE WITH CCC
Original credit reports and/or amended credit reports are routinely returned to the "Client"s home address. "Client" agrees to forward copies of any such reports received by him/her to CCC within five days of his/her receipt. "Client" agrees to provide CCC copies of any documents needed to prove inaccurate and/or negative information within five days of CCC's request (unless good cause exists why a longer period of time is required).
Example: If the credit report reflects that the "Client"s home address is inaccurate, a copy of "Client"s drivers' license may be needed to complete the correction. "Client" would be responsible for providing CCC copies of any documents in his/her possession that would aid her in proving "Client"s assertions.
Failure of "Client" to provide such documentation in a timely manner could result in termination of this AGREEMENT. A reactivation fee may be charged. Documents may be sent to CCC as follows:

865 Cooperlanding Road, PMB 113, Cherry Hill, NJ 08002 - CCC does not guarantee time frames. Each "Client"'s report is different. Each "Client" is given individual attention as demanded by their particular report. However, by law credit reporting agencies have 30 days to verify information and remove erroneous information from your report.
- If negative and/or inaccurate information remains after 120 days, CCC and "Client" will discuss the specific entries and determine the appropriate plan of action. Depending on the action agreed upon, additional fees may be required. Please see the fee schedule below.
- "Client" may cancel CCC's services at any time, however, no refund of the initial payment will be made if CCC has began the agreed upon work, and said work was performed accurately.
- "Client" will be forwarded copies of any documents mailed to any credit bureau by CCC. "Client" will also be provided all passwords, report numbers, and/or other information required for "Client" to access any of his/her credit reports to review any on-line credit disputes.
- CCC agrees not to provide any personal and/or confidential information to any person other than "Client". This includes not providing current phone numbers and addresses to potential bill collectors.
- Administrative set up of "Client" file;
- In depth analysis of "Client"(s) credit reports (TransUnion, Equifax, Experian);
- Postage, copy fees, and any other office expense that may be required to accurately represent "Client".
- The initial fee must be received before services will be initiated.

AGREEMENT OF CCC
CCC agrees to set up "Client"s file and request initial credit reports within 72 hours of receipt of the completed AGREEMENT, information sheet, and fees. CCC agrees to contact "Client" within 48 hours of receipt of the requested reports to discuss reports and plan of action. CCC will dispute negative and/or inaccurate items. After corrected reports are received, CCC will speak with "Client" again to discuss accomplishments and/or need for additional action. CCC will continue this process until all inaccurate and/or negative information is removed. CCC is acting on behalf of ""Client"" with the "Client", collection agency and credit reporting agencies; we are a third party writing disputes on your behalf.
FEE SCHEDULE
The initial set-up fee is $99.00. It includes:
CCC offers three (3) Credit Repair Service Plans: Basic, Intermediate and Advanced
Basic Plan includes the removal of maximum of 10 negative or erroneous items, i.e.:
Set-Up Fee: $99 + $49/month
Intermediate Plan includes the removal of maximum of 20 negative or erroneous items, i.e.:
Set-Up Fee: $99 + $69/month
Advanced Plan is our most comprehensive plan and is suggested when there is severe credit damage. It includes the removal of:
Set-Up Fee: $99 + $99/month *6 month minimum
Additional fees (solely at discretion of CCC):
CCC can also be retained to negotiate and settle a legitimate negative entry between "Client" and any entity which remains on "Client"s credit reports after all efforts described above have failed to remove the negative credit entry.
Example: If "Client" owes $1200 to a credit card that is charged off. A collection agency and/or credit card company may be willing to accept less than $1200 close the matter and mark the "Client"s report as "Paid In Full" making the credit entry positive as opposed to negative. The fee for this service is $150 for claims less than $1000; $250 for claims $1001-$2500; and $500 for claims $2501-$5000. Fees for claims over $500 will be discussed on a case by case basis.
In certain cases, there may be a need to purchase additional copies of credit reports. If this occurs, "Client" will be required to pay for the purchase of any credit reports in excess of the original three (TransUnion, Equifax, Experian). CCC will charge no fees in excess of the $239.00 initial feel without prior approval from "Client".
REPRESENTATIONS
It is understood and agreed that CCC has made no guarantee that all negative entries can be removed from "Client's "s credit reports. Legitimate negative entries cannot be removed from a "Client"s credit report legally. Only time, a conscious effort, and a personal debt repayment plan will improve a credit report. CCC does represent that it will dispute all inaccurate and negative entries and attempt to remove/correct them to the extent allowed by law.
You may cancel this AGREEMENT without penalty or obligation at any time before midnight of the 3rd business day after the date on which you signed the AGREEMENT. See the attached notice of cancellation form for an explanation of this right.
Consumer Credit File Rights Under State and Federal Law
You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor any "credit repair" company or credit repair organization has the right to have accurate, current, and verifiable information removed from your credit report. The credit bureau must remove inaccurate, negative information from your report only if it is over 7 years old. Bankruptcy information can be reported for 10 years.
You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a rental dwelling because of information in your credit report within the preceding 60 days. The credit bureau must provide someone to help you interpret the information in your credit file. You are entitled to receive a free copy of your credit report if you are unemployed and intend to apply for employment in the next 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is inaccurate information in your credit report due to fraud. You have a right to sue a credit repair organization that violates the Credit Repair Organization Act. This law prohibits deceptive practices by credit repair organizations.
You have the right to cancel your AGREEMENT with any credit repair organization for any reason within 3 business days from the date you signed it. Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may occur. You may, on your own, notify a credit bureau in writing that you dispute the accuracy of information in your credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete information. The credit bureau may not charge any fee for this service. Any pertinent information and copies of all documents you have concerning an error should be given to the credit bureau.
If the credit bureau's reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau, to be kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about disputed information with any report it issues about you.
NOTICE OF CANCELLATION
You may cancel this AGREEMENT, without any penalty or obligation, at any time before midnight of the 3rd day which begins after the date the AGREEMENT is signed by you.
To cancel this AGREEMENT, mail or deliver a signed, dated copy of this cancellation notice, or any other written notice to Crystal Clear Credit Management LLC, at 865 Cooperlanding Road, PMB 113, Cherry Hill, NJ 08002 before midnight on the third day after contract is received by Crystal Clear Credit Management, LLC.
